Compensation: Salary $72,000 to $87,000 a year
Nursing and Therapy Services of Colorado (NTSOC) is seeking an experienced and compassionate IHSS RN Coordinator to support high-quality in-home care for pediatric and adult clients. This role coordinates IHSS/Agency-based services, supports individualized care planning, ensures compliance, and serves as a clinical resource for home-based care teams. The coordinator works with leadership and interdisciplinary staff to assess client needs, support service authorization, ongoing eligibility, and training. This position is primarily office-based with regular travel to client homes and is best suited for an RN with home health experience, strong clinical judgment, and a commitment to supporting individuals with disabilities.

Essential Functions:
  • Responsible for the coordination and implementation of all IHSS/Agency-based services, including but not limited to health maintenance activities, personal care services, and homemaker services for both pediatric and adult clients.
  • Coordinates and completes client admissions, ensuring all required assessments and documentation are completed in accordance with regulatory and organizational standards.
  • Provides comprehensive review of client needs to support eligibility determination and service authorization.
  • Works collaboratively with case management to coordinate service authorizations and ensure completion of all required documentation.
  • Participates in the development, implementation, and regular review of individualized care plans, ensuring they are updated to reflect current client needs.
  • Oversees the delivery of IHSS/Agency-based services to clients, ensuring services meet regulatory and organizational standards.
  • Responsible for the training and skills validation checkoffs at initial onboarding and annually for all attendants (personal care workers, health maintenance attendants, and homemakers).
  • Collaborates with the client or authorized representative to troubleshoot any concerns with staff or concerns related to care plans.
  • Tracks and conducts periodic onsite supervision of in-home attendants in compliance with state and federal regulations, completing required documentation in the EHR.
  • Be available for questions from IHSS/CFC attendants with a working knowledge of skilled verses unskilled care.
  • Will cross-train and provide support to Clinical Services with skilled care needs and coverage as operational needs arise.
  • May manage a partial caseload of clients receiving clinical nursing services as needs arise.
  • Participates in a scheduled on-call hotline rotation, providing timely clinical support, triage, and guidance to clients, families, and staff as needed.
  • Other duties as assigned.

Minimum Education: Graduate of an accredited nursing program required; Bachelor of Science in Nursing (BSN) preferred.
Minimum Experience: Minimum of 3 years of RN clinical experience required and a minimum of 2 years of nursing experience in home health or similar healthcare setting preferred. IHSS experience preferred. Strong communication and computer skills required.
Licenses/Certifications: Colorado RN license in good standing with the Department of Regulatory Agencies. BLS required. Up to date car insurance and driver’s license will be required.

Working Conditions:
This is an office-based position, with working hours from Monday through Friday from 8:00 am to 4:30 pm. May complete visits to client homes as needed. May be exposed to occupational hazards such as communicable diseases, harmful chemicals, and/or disoriented or combative clients. Must be able to work under stress and be emotionally capable of coping with changes in activity.
